At Healing with Grace Counseling and Ketamine Center, we believe therapy works best in an environment of respect, clarity, and collaboration. As our client, you have the right to:

• Receive clear, honest information about therapy services, including their purpose, process, potential benefits, limitations, fees, and policies, with the understanding that therapy outcomes vary and cannot be guaranteed.

• Be informed of the potential risks, benefits, and alternatives to therapy and to ask questions at any time so you can make informed choices about your care.

• Actively participate in treatment planning, goal setting, and decision-making in collaboration with your clinician.

• Decline, pause, or withdraw consent to therapy services at any time, except when treatment is court-ordered or otherwise legally required.

• End therapy services at any time, subject to financial obligations already incurred and thoughtful clinical planning when feasible.

• Have your privacy and confidentiality respected within the limits of state and federal law, including HIPAA and 42 CFR Part 2, and to understand the circumstances under which disclosures may be required.

• Request reasonable accommodations for communication preferences or accessibility needs whenever feasible.

• Request a Summary of Services in accordance with clinic policy and applicable law.

• Receive information about your clinician’s credentials, licensure, and supervisory status, and to request referrals or seek a second opinion at any time.
